About the position

The Community Health Specialist II - Tuberculosis position is a vital role within the Lake County Health Department, focusing on the prevention and control of tuberculosis (TB) in the community. Under direct supervision, the specialist will assist in preparing and offering direct individual and community services, client advocacy, and providing both general and specific health interventions in various settings, including community, clinic, and home environments.

 

This position is primarily responsible for conducting TB disease and infection case investigations and surveillance, ensuring that individuals and communities receive the necessary support and education regarding TB prevention and treatment. The specialist will investigate and complete TB case investigations, which includes conducting phone interviews with healthcare providers, schools, and individuals who have or are suspected of having TB infection or disease. They will provide counseling and education to patients and their contacts, sharing crucial information about the disease's etiology and prevention strategies.

 

Additionally, the specialist will enter TB data into the Illinois Disease Surveillance System (IDSS) and other epidemiological software, ensuring accurate tracking and reporting of TB cases. Coordination of daily TB control activities with the TB Coordinator and collaboration with TB staff and other health professionals is essential. The specialist will also prepare and offer direct individual and community services, client advocacy, and health interventions as assigned by the TB Program Coordinator.

 

Establishing and maintaining data on TB disease and infection is a critical function, as is creating and developing statistical analyses and analytical reports to support quality improvement initiatives and performance management. In conjunction with the Epidemiologist, TB Program Coordinator, Public Health Emergency Response Coordinator, and other staff, the specialist will plan and execute readiness exercises to ensure the community is prepared for public health emergencies.

 

Maintaining confidentiality of privileged information and adhering to patient privacy laws is paramount in this role. Participation in staff meetings and assigned training activities is also expected, contributing to the overall effectiveness of the health department's mission.

Responsibilities

  • Investigates and completes TB case investigations.

Requirements

  • Master's degree, Bachelor's degree, or Associate's degree in a related field (Nursing, Public Health, Health Information, Business Systems, etc.).
  • Two years of relevant experience preferred.
  • Valid driver's license with insurance required.
